Unlocking book problem & a gifting ebooks question

I set up my 9 year old daughter with her own account.  She has a Samsung Galaxy Player 5.  I was able to get the book downloaded, but I am stuck at the user name and password.  We have tried everything.  Since the billing is in my name and my cc# I entered that information (just as it shows in the my account area).  I tried with the first letter of my first and last name uppercase and the rest lower case.  I tried with and without a space between my first and last name.  I tried her name.  Nothing is working.  I am using the Aldiko reader to open them.  Any other tips?  I have tried to open the books in ADE and it says it does not match. I am getting frusterated.

Now my other question, if I purchase a book under my user account and gift it to my daugter will we still have to enter a user name and password to unlock it?  TIA

If it is asking for a user name and password, it's looking for the account information, in other words, her user name to log into the BN website, which should be an email address and her BN password.  If it's asking for Name and cc to unlock the book then it's either the name on the credit card or the name on the account, it seems to vary with some folks and the credit card number.  Two different things.  User name and password to log into the account, CC info to unlock the book.
