Can't Remember The Tittle: Mother wrote letters to her unborn son

I read a book about 12 years back where a mother kept a journal or wrote letters to her son when she found out she was pregnet. Then in the end of the book she passed away. Can anyone help me with the tittle.
I think it is something like Letters to or a mothers letters to or journal to. But I have googled it and can not find what I am looking for.
