Children's/YA book about a twin. She's neglected while her sister is spoiled.

I read it a few times but probably not since 5th or 6th grade. She lived in a coastal town or an island town. My mind thinks it seemed like New England. The backstory is that her twin sister was very sickly when they were born so the main character was neglected while her sister was coddled. Years later, the sister is spoiled and the main character is still somewhat secondhand to her. I believe the story happens in the course of a summer but at the very end, the epilogue is of her as an adult. She's a midwife and delivers twin, the second of which is near death. At the end, she goes back to the first twin and tells the parents to hold and love her.
