Its Paul who dies young. Sadie was his wife, and Iris' mother. Sadie marries Alex's father. Alex's father leaves her for another woman.

Yes, Esme is Iris' real grandmother.
How Paul died:
"He was in hospital for a routine operation and he was given a drug he was allergic to. He was young, only thirty-one." (pg. 123)
