Edit a PDF to add Hyperlinks?

I have several PDF files that have chapter indexes but without the hyperlinks to the pages. Is there some way to edit the index of the PDF to add hyperlinks?

If the pdf is DRM protected, probably not.

If you have a license for Adobe Acrobat, (not Acrobat Reader) perhaps you can.
